General comments
vancomycine dosering: oplaaddosis 20 mg/kg, vervolgens 40 mg/kg continu per 24 uur, OF oplaaddosis 30 mg/kg, vervolgens 20 mg/kg 2 dd. Onderhoudsdosering vancomycine verder afhankelijk van nierfunctie en van gemeten spiegels, zie bij vancomycine en bij TDM vancomycine
