followed by

1e recidief, na behandeling met fidaxomicine: extended-pulsed schema.
Dag 1-5: 2dd 200 mg,  dag 7-25 OM DE DAG 1dd 200 mg.

Remarks:

Bij patiënten van wie bekend is dat zij CDI ontwikkelen n.a.v. een bekende trigger, en bij kwetsbare patiënten bij wie het risico op een recidief CDI om een andere reden groot is. 
Secundaire profylaxe te gebruiken vanaf de start van de uitlokkende factor tot 5 dagen na het stoppen van die factor. Keuze middel i.o.m. infectioloog / microbioloog. 

General comments

Stop zo mogelijk verantwoordelijk antibioticum, en isoleer patiënt.

Dosering vancomycine capsules po 4 x 250 mg; 4 x 125 mg suspensie heeft de voorkeur maar is in Nederland niet beschikbaar, eventueel in overleg met apotheek lokale bereiding.
