General comments

Voriconazol: iv 6 mg/kg 2dd gedurende dag 1.

Dosering posaconazol hangt af van formulering:

Therapeutisch:

iv: eerste dag 2 dd 300 mg, daarna 300 mg 1 dd

po: Bij voorkeur tabletten: eerste dag 2 x 300 mg, daarna 300 mg 1 dd po. Alleen indien tabletten onmogelijk: suspensie: 400 mg 2 dd po (met voedsel) of 200 mg 4 dd (zonder voedsel).

Profylaxe:

Bij voorkeur tabletten, eerste dag 2 x 300 mg, daarna 300 mg 1 dd po. Alleen indien tabletten onmogelijk: suspensie 200 mg 3 dd po.

Aspergillus sinusitis, cerebrale aspergillose, pulmonaal aspergilloom met bloeding: chirurgische resectie in combinatie met antifungale therapie.Bij pulmonaal aspergilloom met bloeding arteriële embolisatie overwegen bij inoperabele patiënten.
