General comments
Wanneer na de operatie aan de tractus digestivus antibiotische therapie dient te worden voortgezet doorgaan met:
Link naar doseringen bij obesitas en bij kinderen.
Bariatrische chirurgie
Ingreep met duur < 1 uur:
- Gemiddeld postuur: cefazoline 1 g iv eenmalig
- Morbide obese patiënt (BMI > 40 kg/m2): cefazoline 2 g iv eenmalig
- Bij lichaamsgewicht > 175 kg: overweeg cefazoline 3 g iv eenmalig
Ingreep met duur > 1 uur:
- Gemiddeld postuur: cefazoline 2 g iv eenmalig
- Morbide obese patiënt (BMI > 40 kg/m2): cefazoline 3 g iv eenmalig
Een hernieuwde dosering is nodig bij een operatieduur van >2 keer de t1/2 (deze is voor cefazoline: 1,5 – 2 uur) en bij bloedverlies van > 1500 ml
Toevoeging metronidazol bij bariatrische ingreep afhankelijk van waar wordt geopereerd, bij openen dunne darm: mini gastric bypass / gastric bypass: + metronidazol 500 mg iv eenmalig
