Hemo dialysis: Normale aanvangsdosis, vervolgens interval verlengen op geleide van de spiegel. Op dialysedagen: bij toedienen voor dialyse normale dosis; bij toedienen na de dialyse de helft tot een derde van de dosis.
CAV / VVHD: Interval verlengen op geleide van de spiegel.
